Before Your First Appointment: Please complete and submit the appropriate Training Questionnaire and General Liability Waiver at least 48 hours before your scheduled consultation. If your dog is receiving aggression or reactivity training, please also complete the Aggressive Dog Liability Waiver. All required forms are available on our website.
